Brush Family Medicine offers sports physicals

BRUSH, Colo. (June 6, 2011) – The provider team at Brush Family Medicine is offering sports physicals to middle school and high school students in Morgan County and Woodrow.  The cost of a physical has been reduced for this special event. 

Payment will be on a cash basis only as we will not be billing insurance companies. Physicals will be done on Thursday, June 16 from 8:30 a.m. to 12:00 noon and from 1:00 p.m. to 4:30 p.m. Call Brush Family Medicine at 970-842-2273 (CARE) to register. Registration is requested to help us determine staffing for this event. Physicals will be provided on a first come, first serve basis.

This event is for sports physicals only.  Parents or students requesting physicals for Scout and other camps or for pre-school will be re-scheduled for a different day and the sport physical rate will not apply. 

Athletes must bring a CHSAA health form signed by a parent or guardian. Get the necessary form from your school, CHSAA website or www.BannerHealth.com/EastMorgan.

Do your part to help a family in need, bring canned goods to support our drive for the local food pantry.
