Recognizing the varied needs of our community, Home Health Care offers two options for home health programs based on a doctor's plan of care. Patient and family education are included in the services to promote independence and understanding of prevention, illness and treatment.
The Intermittent Program is designed for patients who are confined to their home for medical reasons and in need of intermittent home visits for a defined period of time. Services include:
- Skilled Nursing - Provides assessments, administration of skilled treatment procedures, patient and family health education, care planning and management
- Physical Therapy - Assists with the development and restoration of function and the maintenance of maximum rehabilitation potential
- Occupational Therapy - Assists with the restoration of daily life activities through prescribed creative activities promoting rehabilitation and independence
- Speech Pathology - Provides diagnostic and therapeutic programs to assist with speech, language, and swallowing disorders
- Medical Social Services - Social workers work with the client and family to develop a community resource plan and to provide long-term care placement assistance
- Home Health Aide - Assists with personal care, activities of daily living, and therapeutic exercises developed by the therapist. All activities carried out are under the supervision of a registered nurse or therapist
- Home Infusion Therapy - Services are provided by a team of trained infusion nurses, pharmacists and other healthcare professionals working under the orders of the attending physicians. Included in the service are equipment, medications, pharmaceutical services, supplies and client/caregiver education. A comprehensive array of infusion therapies can be safely and effectively delivered in the home and are prescribed as a convenient and cost-effective alternative to hospitalization. This service is especially beneficial after surgery and for treatment of chronic conditions.
The Extended Care Program (ECP) is designed for those in need of more continuous maintenance or supportive care to allow them to remain safely in their own home. Examples of ECP services provided include non-skilled wound care, in-home blood draws, medication management, insulin set-up and injection, and maintenance-level home exercise programs. These services are provided and charged on an hourly basis and are generally funded by private insurance contracts, Medicaid waiver programs or private pay.
The FMH Home Health Care Department is Joint Commission on Accreditation of Healthcare Organizations (JCAHO) accredited, and a Medicare/Medicaid provider. Medicare, Medicaid, insurance companies or veteran's benefits may pay for home care services. Please check with your insurer to determine if our home health services are covered.
