Banner – University Medical Center Phoenix's Rehabilitation Institute is located on the Banner – University Medical Center Phoenix campus at 1012 E. Willetta St., Phoenix, AZ.
Outpatient Therapy Services is located on the first floor of the West Wing. We schedule patients from 7 a.m. to 4 p.m., Monday through Friday.
The Rehabilitation Institute is a Medicare provider. To use our services you will need:
- A prescription from your physician (have your physician’s office complete our outpatient referral form.
