At registration, you provided your customer service representative with your email address and selected a four digit PIN (either the last 4 numbers of your social security number or another 4 digit number). As a result, you will receive an email invitation from Banner Casa Grande Medical Center with the subject: Banner Casa Grande Medical Center invites you to join our My Health Patient Portal
- Please follow the link in step 1, which will have you verify your identity by entering your date of birth and either the last four numbers of your SSN or the four digit PIN number that you chose at registration. Once you enter this information, mark the “I agree” box and click on the box that says: Next, Create Your Account
- Now you will need to verify your personal information, reenter your email address, create your user name, select a password and choose your security question/answer. Then, mark the “I agree” box and click on the box that says: Create Account
It is just that simple! Now you will be able to view lab results, allergies we have on file, immunizations given by the hospital, select documents, or obtain a list of medications that have been prescribed to you, as well as view the dates of visits you made to the facility. This allows you to have more access to your health information anytime and anywhere. You may also pay your bill, search our medical staff, update your demographic information, and access general health information.
