Thank you for your interest in applying to the Medical Staff of Banner Estrella Medical Center.

The Application Process

The first step in applying for privileges is to submit the initial application request form.

Upon completion of this form, simply click “Submit Request” and it will be sent via e-mail to the Banner Health Credentialing Verification Organization (CVO). Once received by the CVO, you will be sent a more detailed application to complete. 

The CVO will notify Banner Estrella regarding your interest in applying.  In addition to the application mailed to you by the CVO, Banner Estrella will mail you a facility-specific packet that needs to be completed and returned directly to the Medical Staff Services Department at the Medical Center for your application to be processed.

Minimum Qualifications

The Medical Staff Bylaws (located in Orientation/Resources) describe the minimum qualifications for membership. If, upon review of your application, it is determined you do not meet the minimum qualifications for membership and privileges, your application will not be processed.

Banner Estrella Medical Center requires board certification be obtained within five years of completion of training or sooner as required by the department.
