Pre-Authorization now available at Banner Gateway

Banner Gateway is proud to provide a convenient pre-authorization service for tests you order for your patients in our outpatient medical imaging department.

To initiate the pre-authorization process, which includes scheduling, all you need to is fax the Physician order, facility name, copy of insurance card and patient history and notes to (602) 640-3220.
