Medical Staff Services
The Medical Staff Services Department is available to assist you. The department provides administrative support to the medical staff and acts as a liaison between physicians and administration.

Medical Staff Services is located in the Medical Center just south of the Madera Canyon Conference Room near the Physician's Lounge. Office hours are 7:30 a.m. to 5 p.m., Monday through Friday. Contact the main office by dialing (480) 543-2600 or email the medical staff director.

Banner Health Now Providing Preauthorization for Outpatient Imaging
Banner will now obtain preauthorizations for your patients scheduled for outpatient imaging. Simply fax the patient's insurance card, imaging location, history and notes, and physician order to (602) 640-3220. For stat orders and more information, please call (480) 684-6500. Banner Health is committed to being a convenient resource for your patients' outpatient care.
