• Complete neuropsychological evaluations
• Follow-up short term intervention for review of evaluation results

 Occupational Therapy
Therapists focus on functional daily living skills to improve motor, cognitive and social skills. Services include:

 Physical Therapy
Therapists provide mobility evaluation and training. Services include:

 Speech/Language Pathology
Certified speech-language pathologists evaluate and treat clients to regain communication skills. Services include assessment and therapy for the following conditions:

• Adapted Driving Program
• Seating Evaluation and Prescription
• Pelvic Incontinence

The Banner Good Samaritan Rehabilitation Institute is on the Banner Good Samaritan Medical Center campus at 1012 E. Willetta.

Outpatient Therapy Services is located on the first floor of the West Wing. We schedule patients from 7 a.m. to 4 p.m., Monday through Friday.

The Banner Good Samaritan Rehabilitation Institute is a Medicare provider. To use our services you will need:

• A prescription from your physician (have your physician’s office complete our Outpatient referral form)
• Payor source
