Scheduling an appointment at the Wound Care and Hyperbaric Medicine Center
Please call (602) 839-6040 to schedule an appointment. Our clinic staff will work with your insurance company and your family doctor to assure services are prior–authorized. When you come to the clinic for your first appointment, please bring all of your medications in a zip lock bag or bring a list of your medications.
We will ask you to complete a medical history before seeing the doctor. You may wish to print and complete your medical history form and medication form prior to your appointment.
Insurance authorization for other services
Your doctor may order some tests to help in determining why your wounds are not healing normally. This may include laboratory work, X-rays or other diagnostic studies.
Your doctor may order some services as part of your treatment plan. This may include home health services, consultations with other specialists, therapy services or other supportive care.
The clinic staff will work with your insurance company and your family doctor and will obtain authorization for these services if needed. We will take care of this for you.
Medications and prescriptions
Your specialist may order medications as part of your treatment plan. We can send your prescriptions digitally to your pharmacy of choice. Some medications require an original signature according to state law. These prescriptions can be obtained during your visit to the clinic.
The doctor will want to be aware of any changes in your medications. Please let the staff members doctor know when other doctors have prescribed new medications or discontinued any medications.
Your treatment plan may require you to have dressing changes to your wounds between visits to the clinic. Your insurance may cover the cost of these dressings and many insurance companies have contracted companies who can deliver these dressings to you via mail. We will help to arrange this if it is covered by your insurance. If you do not have this type of insurance benefit, the clinic staff has a list of pharmacies which carry wound supplies.
For best results we follow a team approach. You know your circumstances and your struggles. We have years of experience providing great service and years of experience working with insurance companies and other specialists in the community. Working together ensures the best possible outcome for you.
When you receive care at the Wound Care and Hyperbaric Medicine Center, you will be teamed with a specialist physician and a case manager. The doctor will assess your medical history and the nature of your wound and will work with you and your loved ones to develop a treatment plan that works for you.
The case manager will be an equal partner with you and your physician. The case manager will be available during business hours in case you are experiencing problems between your visits. The case manager will be available for calls to provide direction to resolve problems and can contact the doctor for further direction.
