Banner Heart Hospital's transitionalist, Lynne Rice, MD, continues providing expert one-on-one treatment for patients after they leave the hospital.
She helps patients rehabilitate through a skilled nursing facility, and ensures that patients receive the knowledge, resources and support they need to eventually be successful on their own.
This fresh approach to modern health care aims to eliminate preventable hospital readmissions while humanizing post-hospital patient care.
Dr. Rice works with patients who are at high risk for readmission to the hospital and assesses the practical needs of each patients’ unique case. She then helps the medical teams at the hospital and the skilled nursing facilities to determine what the patient will need to be successful on their journey to recovery.
She combines the expert knowledge and guidance of a doctor with an understanding and resources to take care of the patients’ practical needs. This includes serving as the patients’ primary heart physician while they’re treated at a skilled nursing facility, helping with the transition to the patients’ home, as well as ensuring they have transportation to doctor appointments.
In addition, for patients who do not want to go to a skilled nursing facility, she makes house visits.
