Is it time to consider Banner Hospice?

General Guidelines

• Life-limiting illness determined by your physician or our medical director
• Patient or designated decision maker agree that hospice is the best treatment

Primary Symptoms

•  Desires palliative care (comfort care) vs. curative care
•  Multiple ER or hospital visits
•  Recent decline in functional or nutritional status
•  Weight loss
•  Requires considerable assistance and frequent medical care
•  Dependence in at least three of six activities of daily living (walking, bathing, dressing, feeding, transferring, and toileting)

• Cancer
• Dementia / Alzheimer’s
• Heart Disease
• Liver Disease
• Non-Specific Terminal Illness
• Parkinson’s Disease
• Pulmonary Disease
• Renal Disease
• Stroke and Coma
