Is it time to consider Banner Hospice?
General Guidelines
- Life-limiting illness determined by your physician or our medical director
- Patient or designated decision maker agree that hospice is the best treatment
Primary Symptoms
- Desires palliative care (comfort care) vs. curative care
- Multiple ER or hospital visits
- Recent decline in functional or nutritional status
- Weight loss
- Requires considerable assistance and frequent medical care
- Dependence in at least three of six activities of daily living (walking, bathing, dressing, feeding, transferring, and toileting)

- Amyotrophic Lateral Sclerosis (ALS)
- Cancer
- Dementia / Alzheimer’s
- Heart Disease
- Liver Disease
- Non-Specific Terminal Illness
- Parkinson’s Disease
- Pulmonary Disease
- Renal Disease
- Stroke and Coma
