Caring Connections is Banner's unique program that offers free support and assistance for patients and their families faced with a chronic or life-limiting illness, but not yet ready to elect hospice services.
Through Caring Connections, individuals and their families have access to community resources, crisis intervention, telephone support and education from our staff.

What are the program benefits of Caring Connections?
Caring Connections works with your medical team. We try to educate patients and families about specific disease processes, pain and symptom management and coordinate with the physician.
The Caring Connections program has been found to decrease hospitalizations, increase the resources available to patients and families, and promote effective patient and family coping skills.
How do I get a referral to Caring Connections
Caring Connections is an open referral program. Since Caring Connections is a non-medical assistance program, no physician order is required for service. You can refer yourself by calling (480) 657-1168 or (800) 293-6989. You may also be referred by a family member, friend, social worker, health care provider, nurse or physician.
Once enrolled, you will receive services specific to your needs. Please remember that Caring Connections is a non-medical service.

- Diagnosis of a chronic or life limiting illness
- Frequent physician office visits or recent hospitalizations

How much does Caring Connections cost?
There is no charge for the services provided through Caring Connections. This program is a community service provided by Banner Home Care and Hospice and through the generosity of donations to the Banner Health Foundation.
