At Caring Connections we match patients’ treatments with their goals. The home-based palliative care program is specialized medical care for people with serious illness. This type of care involves addressing the physical, intellectual, emotional, social and spiritual needs - whatever the diagnosis. Home-based palliative care can be provided together with curative treatment. The goal is to improve quality of life for both the patient and family. Caring Connections is part of Banner Health’s Palliative Care Program which provides services throughout Banner medical facilities.
Caring Connections at Banner Home Care will fully integrate the principles of palliative care throughout all phases of patient care in all the communities we serve. Our team works directly with patients' existing physicians and specialists to coordinate a comprehensive care plan based on matching patients’ treatments with their goals.
