The Banner Hospice Team
Our dedicated hospice team is assigned to a specific patient and is available 24 hours a day, 7 days a week. The team collaborates closely with the patient's medical director to create a plan of care driven by the needs and wishes of the patient, family, and other loved ones.

Our medical directors oversee and direct the plan of care a patient receives. They are available seven days a week to care for patient’s needs in their home or at one of the inpatient locations.
The nurse case manager is an expert in pain control and symptom management. They will start a plan of care and continue to assess changing needs, while in constant communication with the medical director.
The social worker addresses the psychological and emotional needs of patient and family, assuring access to community resources.
Home health aides help with personal care (bathing, dressing, grooming) and light housekeeping.
Chaplains are non-denominational and can provide spiritual support for the patient and their family.
Trained volunteers provide companionship, friendly visits, and also help caregivers.
Registered dieticians help with nutrition.
A bereavement counselor provides support for up to one year after the patient's death. In addition, provides anticipatory grief throughout the time the patient is on hospice services.
Physical, occupational, respiratory and speech therapies are available when deemed necessary by the team.

- Dog Therapy consists of trained and certified dogs that will make special visits to encourage smiles and pain relief.
