In order for Iron Mountain to provide copies of medical records to patients, an authorization signed by the patient or legal representative is required. Please download one of the forms below or call Iron Mountain at (602) 252-1570.
Release Forms
Upon receipt of a completed authorization form, copies of all pertinent portions of the medical record will be made and forwarded to the address provided on the authorization form. This process will normally take five to seven business days from the date Iron Mountain receives a request. Iron Mountain may require payment in advance to cover the costs of the clinical abstract of pertinent data.
