• Banner Good Samaritan Occupational Health Clinic is open seven days a week.
  • The hours of operation are 6 a.m. to 10 p.m., Monday-Friday, and 8 a.m. to 4 p.m Saturday-Sunday and the phone number is (602) 839-4456.
  • Please note that all phone numbers at the Banner Good Samaritan Clinic now have the 839 prefix (formerly 239).
• Banner Gateway Clinic is open 7 a.m. to 6 p.m., Monday-Friday and 8 a.m. to 4 p.m. on Saturdays and the phone number is (480) 543-3300.  

Banner Gateway Clinic now in new location
Banner Gateway Occupational Health Clinic has moved across the campus at Banner Gateway Medical Center into the new medical office building at 1920 N. Higley Road, Suite 108. The hours of operation will remain the same as well as the phone and fax numbers.
