• Banner Good Samaritan Occupational Health Clinic is open seven days a week.
  • The hours of operation are 6 a.m. to 10 p.m., Monday-Friday, and 8 a.m. to 4 p.m Saturday-Sunday and the phone number is (602) 839-4456. All clinics are closed on holidays.

Banner Gateway Clinic now in new location
Banner Gateway Occupational Health Clinic has moved across the campus at Banner Gateway Medical Center into the new medical office building at 1920 N. Higley Road, Suite 108. The hours of operation will remain the same as well as the phone and fax numbers.
