As part of this program, you will be asked to do one or more of these activities each day:
- Answer questions about how you feel
- Measure your own weight, blood pressure, heart rate, glucose level or blood oxygen level using small devices set up in your home.
It's easy to do and you can contribute significantly to a healthy lifestyle. Your care team receives daily reports about your health. Daily information serves as a useful supplement, not a replacement to the care from your doctor.
Your health care provider will deliver everything you need to your home and teach you how to use it. The system is designed so you can take the measurements by yourself. Results are sent automatically through your telephone line, toll free, to a health care professional who reviews them daily. There are no interruptions to your phone service.
If a significant change occurs in your measurements, the reviewing health care provider will call you or your doctor.
