The Anticoagulation clinic team consists of friendly, highly trained clinical pharmacists, nurse practicioners, and phlebotomists.
The Anticoagulation team is here to provide you with a cost effective, convenient, and coordinated care experience. The team is dedicated to providing quality service, care, and education.
Services Provided by the Summit View Anticoagulation Clinic
- Monitor, adjust and evaluate Coumadin® (warfarin) therapy.
- Perform fingerstick blood test to monitor therapy.
- Provide Coumadin® (warfarin) prescriptions for refills.
- Write blood draw lab orders during travel or vacations.
- Monitor and adjust Coumadin® (warfarin) therapy and/or add Lovenox® during procedures (dental procedures, colonoscopy, hip surgeries, etc)
- Provide anticoagulation care to patients during travel or home health care treatment.
- Consistent communication and discussion with your physician to provide best care.
