The Anticoagulation clinic team consists of friendly, highly trained clinical pharmacists, nurse practicioners, and phlebotomists.
The Anticoagulation team is here to provide you with a cost effective, convenient, and coordinated care experience. The team is dedicated to providing quality service, care, and education.
 
Services Provided by the Summit View Anticoagulation Clinic

  1. Monitor, adjust and evaluate Coumadin® (warfarin)  therapy.
  2. Perform fingerstick blood test to monitor therapy.
  3. Provide Coumadin® (warfarin) prescriptions for refills.
  4. Write blood draw lab orders during travel or vacations.
  5. Monitor and adjust Coumadin® (warfarin) therapy and/or add Lovenox® during procedures (dental procedures, colonoscopy, hip surgeries, etc)
  6. Provide anticoagulation care to patients during travel or home health care treatment.
  7. Consistent communication and discussion with your physician to provide best care.

About the Anticoagulation Clinic

The word anticoagulation means "to thin the blood".  Anti coagulation can be achieved by injections of heparin or Lovenox® or by oral medication Coumadin® (warfarin).  The Anticoagulation Clinic was developed by physicians and pharmacists for patients who are on blood thinning medication.  Each time you visit the clinic, a blood sample will be obtained.  This is usually done by a finger stick.  This blood will be tested to check how well the medication is working.  Results will be available before you leave the clinic.  The clinician will tell you how much medication you need to take and order anticoagulation medication for you.  The clinician will update your physician after each visit.  You no longer have to wait for lab results, physician visits, and telephone calls to change your medication dose.

Anyone who is referred by their physician may come to the clinic. If you are interested in attending the clinic please call us and discuss with you physician.  The physician's office will contact the clinic and make your first appointment.  Please bring a list of all your current medications to your first visit.
