Blood Chemistry Instructions
If you are planning to participate in the Blood Chemistry Analysis Blood Screening (Chem-24), remember that you should fast for at least 10-12 hours, but no more than 16 hours prior to your scheduled appointment. During the fasting period, drink plenty of water and take all prescribed medications. Avoid excessive exercise prior to your blood draw appointment. Contact your physician with any medical concerns regarding fasting. Fasting is not required for the CBC, PSA or hs-CRP Blood Screenings.
Participants With Diabetes
If you have diabetes and will be having your blood chemistry analysis done, please eat normally and take all medications, including any diabetic medications. Do not fast! Your test results may show elevated levels of glucose, cholesterol and triglycerides due to non-fasting.
You should drink plenty of water to avoid dehydration!
Directions for filling out the Personal Wellness Profile
Please complete - in pencil - the Personal Wellness Profile questionnaire and bring it with you to your scheduled appointment.
Note:
- Group ID Number is the date of the event: MM-DD-YY.
- Personal ID Number is your birthday with a zero ("0") in front. For example, if your birthday is September 28, 1983 or 09-28-1983, your Personal ID Number would be 009281983.
Please take note and use the same number year to year so you can view your trend report. If you do not remember your Personal ID Number, please notify the staff at check-in.
Results
We encourage all of our health fair participants to share the results of your blood screening with your physician during your next annual appointment.
Results will be mailed to your home address in 1-2 weeks.
If your company opted to include a Health Risk Assessment, results will be mailed in 3-4 weeks. If you do not receive your results within this time frame, please call (970) 378-4772.
Follow-up
If any of your health fair screenings result in an abnormal range, you will be asked to follow up with our own physician.

If you need to cancel or reschedule your appointment, please let your employer contact know.
