Weld County Paramedic Services employees are to use the Incident Report Form to document an Incident Report where any occurrences or unprofessional conduct that fall outside the normal safety, operational, or medical procedures of the Paramedic Service.
Additionally Weld County Paramedic Services employees are to use the Incident Report Form to document an Incident Report where equipment or vehicles are found to be broken, damaged or malfunctioning.
If there is any question on the type of situation that may need to be documented on the Incident Report Form, contact the on duty Supervisor immediately.
The incident report must be generated as soon as possible by all employees involved in the incident in order to gather as much information by all parties involved in the incident.
When the employee has completed the incident report, they should save a copy of it to their local drive and then e-mail it as an attachment to the appropriate e-mail group. This must be done prior to the end of your shift and the employee leaving work.
