Vehicle Checklist

 

Banner Health / NCMC Paramedic Services employees are to use the link below to complete the Daily Vehicle Checklist of their ambulance at the beginning of their shift.

Procedure:
The Daily Vehicle Checklist must be completed at the beginning of each shift or whenever a crew is moved into another ambulance for any reason.

All fields on the Daily Vehicle Checklist must be filled in and all items on the Daily Vehicle Checklist must be present and accounted for.  For any items that are not present and accounted for, this must be noted in the comments section at the bottom of the checklist, the on duty Medical Operations Supervisor must be notified and an incident report must be generated and e-mailed to the appropriate e-mail group.

For those fields on the Daily Vehicle Checklist that require a numerical value (i.e. monitor number, glucometer readings, and oxygen p.s.i. levels) please document the appropriate numerical value.

For those fields regarding the operational aspect of the vehicle, all parts should be found in working order.  For any parts not found to be in working order, this must be noted in the comments section at the bottom of the checklist, the on duty Medical Operations Supervisor must be notified and an incident report must be generated and e-mailed to the appropriate e-mail group.

When the employee has completed the Daily Vehicle Checklist, they are to save a copy of it to their local drive and then e-mail it as an attachment to the AM – Supervisors e-mail group where a copy of it will be saved on the computer in the Supervisors folder.
