Community Wellness Package
The first step to taking care of your health is knowledge. At Community Hospital we make it easy to know your numbers and monitor your progress.
Every weekday, Community Hospital staff is available to help you get the screenings and tests you and your family need to ensure you are on the right track to your healthy best.
Many tests and screenings require no appointment, but be sure to check if fasting is required.
The Wellness Package at Community Hospital is designed to provide the tools and support to help you take responsibility for the health and wellness of yourself and your family.
If you have any results that fall out of the normal range, you will be contacted by the Community Hospital Wellness Nurse who can answer questions, and will encourage follow-up with your physician.
Walk-in service is Monday-Friday, 7 a.m.-3 p.m.
Payment for service is expected up-front. Insurance will not be filed but may apply to any follow-up testing that may be required.
