Program Requirements:

• Pharm D degree from an ACCP accredited college of pharmacy
• Current Arizona pharmacy license (or eligible for)
• ASHP Match Registration

Application Instructions

• Residency candidates should submit all required documents through the Pharmacy Online Residency Centralized Application Service (PhORCAS) , no later than January 15th.
• Application materials must be received prior to candidate interview(s)
• Selected candidates will be contacted to schedule an on-site interview in January or February
• An on-site interview is required
