How to Schedule an Appointment

To make an appointment at the Memory Disorders Clinic, please call (602) 839-6900.

During your first phone call:

• We will ask for some preliminary information
• Make arrangements to have medical records sent to our clinic
• Send a health questionnaire to fill out before the first visit.

Fill out forms ahead of time to make your visit go smoothly  

Referrals and Billing
The clinic's care providers work collaboratively with primary care physicians and/or referring specialists.

Some insurers may require a referral and some services may fall under the behavioral health portion of the contract.

Please contact the patient's insurer to determine if a referral is necessary. If so, the patient or family should contact the primary care physician to obtain a referral and have the consultation request completed by the referring physician.
