Physician Referral and Information
As leaders and experts in memory disorders and dementia, our team provides comprehensive consultation and evaluations for patients with cognitive concerns. Our 90 minute initial evaluation includes a cognitive battery, detailed history, and physical examination. Laboratory tests, brain imaging or other related tests will be ordered as required to complete the evaluation.
Once our physicians make a diagnosis, they explain the findings and craft a treatment plan which is shared with the patient, the patient’s family/caregivers, and the referring physician. Treatment plans include both pharmacologic and family and community services therapies. Patients and caregivers will also have opportunities to participate in leading-edge research including studies of experimental treatments in clinical trials.
Our medical staff works collaboratively with primary care physicians and/or referring specialists. While most patients and family choose to continue to have their dementia-related needs attended to at Banner Alzheimer's Institute, we are happy to provide a comprehensive plan for the primary care physician to execute the plan of care. All non-dementia related medical issues are deferred to the primary care physician.

Our providers are contracted with most major payers; some insurance plans may require a referral or prior authorization, and some services may fall under the behavioral health portion of the contract.
