School-Based Health Centers
At Banner Health, we exist to make a difference in people’s lives through excellent patient care.
That is why we provide health services to uninsured students in the Phoenix Metropolitan area through our School-based Clinics and Banner Children’s Healthmobile.
Our goal is to keep your child healthy, and limit his/her number of absentee days from school. This service is efficient, cost effective, and easily accessible through your school nurse.
- Health assessment and health history
- Well child checks
- Health promotion
- Diagnosis and treatment of episodic illness
- Sports and camp physicals
- Referral to appropriate specialty care; cost of specialty services are not covered by Banner School-Based Health Centers
Eligibility
- Your child must be enrolled in a Phoenix Metropolitan school.
- If you have private insurance, AHCCCS, or KidsCare, you must see your selected primary care provider.
- If you have applied for AHCCCS and are waiting for services, your child can be seen for a limited time.
- If you can provide us with an AHCCCS denial letter, we will continue service.
- Our clinics do not provide emergency care, hospitalization or trauma care. We also do not provide dental care, reproductive health care or mental health care, but can provide more information on resources available. Contact your school nurse, who will assist you in making an appointment.
School-based Clinics
Our health clinics are located at several schools around the Valley. Students do not need to attend these schools in order to receive care. Please speak to your school nurse about the clinic closest to you.

Banner Children’s Healthmobile
Banner Children’s provides health services to uninsured students in the Phoenix area via our healthmobile. This service is free and easily accessible by calling (480) 412-6344 for an appointment. A bi-lingual scheduler will be available to assist you.
