Accepted Health Insurance
Banner Health accepts most major insurances. If you have any questions about coverage, please contact your insurer prior to scheduling an appointment to confirm your physician's participation with your plan.
Fees & Payments
We find that communication with our patients regarding our financial policy helps us provide the best of service to you. If you have medical insurance, we will offer our assistance in helping you to receive your maximum benefits. To do that, we need your help and understanding.
As medical providers, our relationship is with you, not your insurance company. All charges are your responsibility. You will be requested to pay your co-payment at the time of the service. In some cases, we may be able to determine your coinsurance and deductible. In that case, coinsurance and deductibles are due at the time of service.
If your insurer contracts with us, we will bill them for you. If you fail to provide the required insurance information needed to file a claim on your behalf, you will be responsible for your balance. We will require detailed information and a copy of your insurance card to provide this service. Most insurers require referral or preauthorization for medical services. A copy of your card will help us to obtain these necessary approvals from your insurer. Obtaining authorization is not a guarantee of payment by your insurance.
Charges for medical services are due and payable at the time of service. A Patient Financial Services Representative will be made available if other financial arrangements are required. We do accept Visa, MasterCard and Discover. Accounts unpaid 90 days past the date of service are considered delinquent and are subject to collection proceedings. All checks returned for insufficient funds are assessed a fee of $40 by PFC Check Solutions.

If your account is self-pay (no insurance) and you pay at the time of service, we will allow a 20 percent discount on services billed.
Global Periods after Surgery
The majority of orthopedic surgical procedures carry a 90-day global period. This means that for 90 days from the date of your surgery, office visits are included in the cost of the surgical charge. Office visit copays are not required during this 90-day time period. Other services, including X-rays, cast applications, or injections are billable procedures during the global period.
X-rays and Durable Medical Equipment
Although you may receive an x-ray or some type of durable medical equipment at your visit in our office, these services are not billed by Mountain Vista Orthopaedics. You should expect to receive separate bills for each of these services if they are necessary at your visit.
