Second Chance to Enroll for 2010 Benefits
Your second chance to make benefit elections for next year will begin on Monday, Nov. 16 and end on Tuesday, Nov. 24.
A confirmation statement showing your 2010 benefit choices will be mailed to your home beginning Nov. 12. Please review your confirmation statement carefully; the benefits listed should match your final selections during the Annual Enrollment period, or those that rollover automatically from 2009. Flexible Spending Accounts (FSAs) and Medical Waiver credits don’t roll over automatically, so if you forgot to make your FSA elections or find a change on your confirmation form that you would like to make, use this second chance to get the benefits you want for 2010.

Dependent Verification If you are adding a new dependent to the plans, you will be required to submit verification of their eligibility before they will be added to the plan(s). You'll find the required forms with your enrollment confirmation statement after you enroll. |
