Your Banner Medical Plan and Greeley Medical Clinic – Poudre Valley Medical Group Effective April 1, 2012, the Greeley Medical Clinic - Poudre Valley Medical Group (GMC – PVMG) will move to the Indemnity level of coverage under the Choice Plus plan and to the Non-network level of coverage under the Select plans. Need to Order Additional Medical/Dental ID Cards? To order additional or replacement ID cards, log into Banner Health Plans and follow the instructions on the “Get an ID Card” page. The card(s) will be sent to you within 14 days. If you need access to an ID immediately, you can also print a temporary card from the Banner Health Plans site. |

2012 Banner Pharmacy Plan and Walgreens As previously communicated, Walgreens is no longer participating in Banner’s pharmacy plan networks as of January 2012. You will be responsible for paying 100% of the costs of your prescription medications out of your own pocket if you continue to use Walgreen’s pharmacy services in 2012. If you have not already done so, take time now to transfer your prescriptions to a network pharmacy to assure that there is no interruption in your medications. |
