Hyatt Legal Plan
The Hyatt Legal Plan provides benefits for you and your eligible dependents through easy and low cost access to a wide variety of personal legal services. Enrollment is required before coverage can begin. You pay the full cost of this coverage. Your cost for any coverage you elect is collected from your paycheck on a post-tax basis.
Who is eligible?
You are eligible for legal coverage if you are scheduled to work 32 hours or more per pay period and are classified as a regular full-time or part-time employee.

What are the choices for coverage?
You can either choose to purchase legal coverage or elect no coverage. Your cost for coverage does not depend on how many eligible dependents you have.
How do you enroll?
You make your legal plan coverage election during your eligibility period or during the Annual Enrollment period. You make your coverage choices through the Banner Benefits Administration System by clicking on the "Enroll for Benefits" in the right hand corner of this page or by calling Banner Plan Administration at 1-800-827-2464 or in the Phoenix Metro area 480-827-7070. You will receive a confirmation statement of your legal plan elections.
Your coverage begins the first of the month following date of hire or movement into an eligible employment position. If you wish to enroll at a later date, you may enroll during the Annual Enrollment period for an effective date of January 1st.
What is your cost for coverage?
Your coverage cost is collected with post-tax contributions over the year through 24 regular deductions from your paycheck. When enrolling in the plan you are required to stay in the plan for the entire year you elect coverage.
What are the legal plan benefits?
The legal plan gives you and your dependents access to professional legal representation at an affordable price through Hyatt Legal. The plan features:
In-network benefits where all covered services are received from a Hyatt network legal provider and are paid in full and you have no claims to file or Out-of-network benefits where you may choose a legal provider outside the Hyatt network and be reimbursed using a fee schedule.

The following is a listing of the personal legal services covered by the legal plan when you choose a network legal provider:

- Powers of attorney
- Review of personal legal documents
- Consumer protection matters
- Debt collection defense
- Identity Theft Defense
- Personal bankruptcy
- Small claims assistance
- Uncontested adoption
- Uncontested guardianship & conservatorship
- Tenant and eviction problems (tenant only)
- Name change
- Pre-marital agreement
- Restoration of driver's license (except DUI)
- Living trusts (not tax planning)
- Document preparation such as deeds, mortgages, notes, demand letters, affidavits
- Immigration Assistance
In-network discounts are available on probate and personal injury. Office consultation and telephone advice is available on virtually any personal legal matter. To find a participating attorney in your area, go to the Hyatt Legal website and use password 3090010.
What types of services are excluded?
Excluded from coverage are any business-related matters including rental property, and issues relating to your employer, appeals, class action suits, and any matter where a spouse or dependent's interest might conflict with yours. The legal plan fully covers in-network attorney fees for covered services, but does not cover any third-party costs such as filing fees, recording costs, depositions, fines, and court costs.
Coverage is not available under the plan in cases where you are currently receiving or have received legal services before your enrollment is effective or before you have received a case number from Hyatt Legal Plans.
What if your legal problem is not covered?
If you don't find your type of case in the Types of Services Included or Types of Services Excluded described above, the plan covers your initial consultation. Some common examples are divorce, tax problems or child support. After the covered consultation, you may need representation that is not covered by the Plan. If you hire a network attorney to handle a non-covered case, the attorney will tell you what the fee will be before your case begins.
Where can you find more information?
Hyatt Legal encourages potential enrollees to call the Client Service Center during the enrollment process. This can provide a high comfort level for employees who are in the process of making an enrollment choice. The Hyatt Legal reps will be happy to answer questions concerning the location of plan attorneys, how to go out of network, etc. Hyatt Legal Plan reps will also mail or fax definitions of covered services to employees who want more detailed information.
Therefore, all Banner Health employees (enrolled or not) have access to Hyatt Legal's toll free number 1-800-821-6400 (Monday 8 a.m. - 8 p.m., Tuesday - Thursday 8 a.m. - 7 p.m. and Friday 8 a.m - 6 p.m. Eastern Time). A current list of attorneys will be mailed to you upon request. You may also visit the web site at http://www.legalplans.com/ (use Password 3090010).
A Hyatt Client Service Representative will confirm that you are enrolled, give you a case number and the names, addresses and phone numbers of nearby network attorneys. You then call the attorney to set an appointment. Most network attorneys schedule appointments in the evenings or on Saturdays, so you don't have to miss work. If you wish, you may also choose an out-of-network attorney and receive reimbursement for covered services according to a set schedule of fees.
