The Service Center at Banner Plan Administration administers many of your employee benefits.

Contact the Service Center to:

  • Enroll for your benefits
  • Make changes to your benefits due to a qualifying event
  • Ask questions about Banner’s benefit plans
  • Ask about Banner contracted doctors and other health care providers for the Choice Plus and Select medical plans and the dental plans
  • Ask about authorizations and claims for your Choice Plus or Select medical plan or the dental plans
  • Request additional or replacement cards for the Choice Plus or Select medical plans or the dental plans.

For provider, authorization, claims or card information for the Banner Health Savings medical plan, contact Blue Cross Blue Shield of Arizona by calling (855) 801-4637 or visiting the Blue Cross Blue Shield of Arizona website.

  • Office Hours: 7 a.m. – 6 p.m. Arizona time (Monday - Friday)
  • Email the Service Center at Banner Plan Administration with questions and/or feedback, and a staff member will respond within two business days. (Monday - Friday).
