You are considered to be a “benefits eligible” employee if you are regularly scheduled to work 32 hours or more per pay period and are classified as a full-time or part-time employee. Full-time employees are scheduled to work 64 or more hours per pay period on a continuous basis. Part-time employees are scheduled to work a minimum of 32 hours but less than 64 hours per pay period on a continuous basis.

You may elect to participate in the following benefits by completing the enrollment process. With the exception of the legal and retirement plans, you may also choose to cover your eligible dependents under these plans. |

Provided you are actively at work, Banner Health automatically provides this coverage beginning on the first day following 365 calendar days in a regular full-time or part-time employment classification. If a break in service or change to an ineligible employment classification occurs prior to your eligibility or after you were eligible, other rules apply and are located in the Benefit Summary. | |

Travelers are employees who work short-term assignments throughout Banner Health’s facilities in Alaska, Arizona, California, Colorado, Nebraska, Nevada and Wyoming.

Becoming Eligible: In the event your employment status changes to regular full-time or part-time and you become eligible for benefits, you will have 31 days from the date of the status change to enroll..
Losing Eligibility: If you move to an employment status that is not benefits eligible, or you leave employment with Banner Health, your medical, pharmacy, dental, vision and Flexible Spending Accounts coverage for you and your qualified covered dependents may be extended through the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA). For additional information on COBRA, please read through the COBRA Notice of Rights and/or contact the COBRA Administrator, Discovery Benefits, at (866) 451-3399.
