Employee Eligibility  

At Banner Health, we offer a variety of employment statuses, pay programs and benefit plans that support your Journey. Your employment status determines your eligibility for benefits. Details about employment statuses can be found in the Employee Handbook on the internal employee website.

Benefits for Full- and Part-Time Employees

You are considered to be a “benefits eligible” employee if you are regularly scheduled to work 32 hours or more per pay period and are classified as a  full-time or part-time employee. Full-time employees are scheduled to work 64 or more hours per pay period on a continuous basis. Part-time employees are scheduled to work a minimum of 32 hours but less than 64 hours per pay period on a continuous basis.

 

Banner Health will automatically provide you with these benefits upon hire/transfer into a benefits-eligible full-time or part-time position.

 

You may elect to participate in the following benefits by completing the enrollment process. With the exception of the legal and retirement plans, you may also choose to cover your eligible dependents under these plans.


As a condition of participation in Banner's medical plans, employees in Arizona and Colorado are required to sign an authorization allowing Banner to collect overdue payments for Banner services through payroll deduction when necessary. Banner employees located in other states are not required to sign this authorization at this time. Enrollment in other benefits is not affected.

Provided you are actively at work, Banner Health automatically provides this coverage beginning on the first day following 365 calendar days of employment classification.

If a break in service occurs prior to your eligibility or after you were eligible, other rules apply and are located in the Benefit Summary.

Benefits for Travelers

Travelers are employees who work short-term assignments throughout Banner Health’s facilities in Alaska, Arizona, California, Colorado, Nebraska, Nevada and Wyoming.

Banner Health will automatically provide you with these benefits upon hire/transfer into a benefits-eligible Traveler position.

You may elect to participate in the following benefits by completing the enrollment process. With the exception of the legal and retirement plans, you may also choose to cover your eligible dependents under these plans.

Changes in Eligibility

Becoming Eligible: In the event your employment status changes to regular full-time or part-time and you become eligible for benefits, you will have 31 days from the date of the status change to enroll.

Losing Eligibility: If you move to an employment  status that is not benefits eligible, or you leave employment with Banner Health, your medical, dental, vision and Flexible Spending Accounts coverage for you and your qualified covered dependents may be extended through the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA).  For additional information on COBRA, please read through the COBRA Notice of Rights and/or contact the COBRA Administrator, Discovery Benefits, at (866) 451-3399.
