Benefits for Full/Part Time Employees
You are eligible for Banner Benefits if you are scheduled to work 32 hours or more per pay period and are classified as a regular full-time or part-time employee. In the event your status changes from non-benefits eligible to benefits eligible, you have 31 days from the status change to enroll in benefits.
Employment statuses used by Banner Health include:
Benefits Eligible Employees:
- FT (Full-time): regularly scheduled for 32 hours or more per week
- PT (Part-time): regularly scheduled for 16-31 hours per week
Other Employees:
- PIL (Pay In Lieu): regularly scheduled for 16 hours or more per week, paid an additional amount instead of benefits
- Registry: receive a flat rate, ineligible for most benefits
- Weekend Staffing: regularly scheduled for 24 hours per weekend on a continuous basis, paid an additional
amount for each hour worked - Additional employment statuses may be offered; benefits eligibility may vary
Eligible Dependents include your:
- Legal Spouse
- Qualified Domestic Partner
- Unmarried child or children up to age 26
Proof of dependent eligibility will be required during enrollment.
If you would like to cover a Domestic Partner, you must complete and fax the Benefits Change/Enrollment Form to the Service Center at Banner Plan Administration . The fax number is located on the form.
