How To Enroll
You make your coverage elections during your initial eligibility period or during the Annual Enrollment period. You will receive a price sheet showing the options available to you, including your cost of coverage.
Below are the instructions to enroll for your Medical, Pharmacy, Dental, Vision, Flexible Spending Accounts, Life/Accidental Death and Dismemberment and the Hyatt Legal Plans on-line or by phone:
Step 1: Read your Benefits Enrollment Guide for an overview of Banner Benefits.
Step 2: Decide whom you want to cover. You can choose from these coverage levels:
- Yourself only
- You and your spouse or domestic partner
Domestic Partners (DP) and their dependents cannot be enrolled online. When enrolling online, please proceed with your own enrollment. For DP enrollment, complete the DP Packet, then contact the Service Center at Banner Plan Administration by calling (480) 684-7070 in the Phoenix-metropolitan area or (800) 827-2464 in all other areas.

Step 3: Choose the benefits you want and assess your costs.
Step 4: Enroll for your benefits.
You will need to have the following information available to you when you enroll on-line or by phone.
- Your employee ID number
- Your eligible dependent full names, dates of birth and Social Security numbers
- Your beneficiary/ies full names, dates of birth, addresses and Social Security numbers
- Your PCP (Primary Care Physician) name and ID number for yourself and your dependents, if you are enrolling in the Choice Plus Medical Plan. The required physician information may be obtained through the Provider Search
Please Note: Banner Health maintains the right to periodically request proof of eligibility.
You can enroll in one of two ways:
- On-line
- Go to https://www.buckhrsolutions.com/banner/emplogon.asp.
- Enter your Employee ID number
- Enter your eight digit Personal Identification Number (PIN), which is your date of birth. For example, if you were born Jul. 17, 1958, your PIN would be 07171958.
- You will need to follow the instructions and create your own unique PIN once you have logged into the enrollment system.

If your dependents are currently enrolled in the Plans, ensure their social security numbers are correct in the enrollment system. Once in the enrollment system, there is an “Edit” button next to their social security numbers that you must select to update their social security numbers.

If you are adding new dependents to the Plans during Annual Enrollment, you must supply their social security numbers during the enrollment process. After Annual Enrollment closes, you will receive the dependent verification packet with your confirmation statement. You must return the Verification of Eligibility for Dependent Form along with the supporting documentation (i.e. Tax form, birth certificate, marriage license, etc.) before your new dependents will be enrolled in the plan.
