2013 Biometrics Incentive

ECHO (Employees Choosing Healthy Options) is ready to help you Know Your Numbers with a free biometrics screening. The 15-minute screening and a free blood test are offered annually to every Banner employee.
As part of our ongoing commitment to employee wellness, personal accountability and health management, Banner Health will again offer an incentive in 2013 for individuals who complete a biometrics screening by certain deadlines in 2012. The 2013 incentives are:
- If you enroll in a Banner medical plan for 2013, completion of your 2012 biometrics screening makes you eligible to receive a $10 per month premium offset for 2013. If you have already completed a screening in 2012 you have met the requirements to receive the 2013 incentive; no further action is needed.
- If you do not enroll in a Banner medical plan for 2013, you will receive the 2013 incentive as a one-time $50 payroll credit in late January 2013 instead of a premium offset
If you haven’t yet completed a biometrics screening in 2012 and wish to receive the 2013 wellness incentive, you must complete a screening, including required blood work, by the December 2012 deadlines listed below. If you do not, you will miss out on the 2013 incentive, and your next opportunity to participate will be for incentives offered for 2014.
You have two options for completing biometrics:
- Free screenings are being offered by members of our ECHO team. Under this option, the physical portion of the screening must be completed by Dec. 14, and your blood draw must be completed by Dec. 19, 2012.
- Your primary care physician may complete your biometrics screening and blood draw. You will be responsible for all costs, and for collecting and submitting all completed paperwork. Your deadline to submit all results from your physician screening is Dec. 19, 2012.
