Durable Medical Equipment

To assure quality, cost-effective medical equipment coverage for our medical plan members, Banner Health has created a network of contracted Durable Medical Equipment (DME) providers to assist you and your covered family members with your DME needs.

All DME must be obtained through a contracted provider. DME from any non-contracted provider will no longer be covered under the plans.
