Medical Plans
Banner Medical Plans provide benefits to you and your eligible dependents for illness, injuries, wellness and preventive care. Enrollment is required before coverage can begin.
What are the Choices for Coverage?
Banner offers the following choices for your medical coverage:
- Banner Select $250
- Banner Select $500
- Banner Select $1000
- Banner Select Choice Plus
- Waive Coverage or Waive with Proof

Waive Coverage or Waive with Proof
If you do not wish to enroll in a Banner Medical Plan, you may waive coverage. If you have medical coverage elsewhere, you can choose to waive Banner Medical Plan coverage and receive a monthly credit to use toward the cost of your other benefit choices.
To receive the credit, you must elect to waive coverage, and then provide proof of other medical coverage by completing a Medical Waiver form.
You must complete and submit a new Medical Waiver form each year.
