Banner Pharmacy Plans - Prescription drugs to help you stop smoking are covered for all members enrolled in one of the Banner Pharmacy Plans. Contact the Service Center at Banner Plan Administration by calling (480) 684-7070 in the Phoenix metropolitan area or (800) 827-2464 in all other areas.
