Banner Health offers a variety of benefit options and resources outside of the regular benefit package so you can tailor a program that makes the best choices for you and your family.

To enroll for the following Personal Plans, you must either attend a one-to-one meeting with a benefits counselor at your facility or visit their website (Personal Plans for Banner Health) to start your review of these benefits. Because these are individual insurance plans, you will need to meet with a Counselor in order to activate the plans you wish to participate in.

All of the Counselors have been specially trained on Banner Health's benefit plans.Whenyou meet with your counselor, you will learn about the features and benefits of each new voluntary product so you can decide what types of coverage are right for you and your family. This consultative approach will provide you the opportunity to ask questions about these benefits to ensure you have the information you need to make informed decisions about your personal welfare and that of your family.

• Individual Universal Life Insurance with Long Term Care Rider:Permanent life insurance that includes a cash value feature and long term care coverage option
• Critical Illness Insurance with Cancer Rider: Upon diagnosis of a covered critical illness, you receive a lump-sum benefit to use however you want.
• Accident Insurance: Provides benefits when accidents happen to cover the costs health insurance doesn't.
• Auto Insurance: Continued to be offered through MetLife, you can get attractive auto insurance rates. Ask you current carrier to quote on the same basis and compare the savings.
• Home Owner's Insurance :As with the Auto Insurance rates, Banner Employees have seen savings when comparing rates to current carriers.
• Renter's Insurance: Often overlooked by Renters, this insurance provides security against loss of personal property while renting. It's comprehensive and affordable.
• Pet Insurance: Covers thousands of medical problems and conditions for your dogs, cats, birds, rabbits, reptiles and other exotic pets. Vaccination and routine care coverage is also available.
• Computer Purchase Assistance: Provides a convenient and affordable way to purchase quality, name-brand personal computer bundles and related products and services, regardless of credit status and with the convenience of payroll deduction.
• Mortgage Assistance: A dedicated team of experts is ready to help you buy, build or refinance a primary residence, vacation home, or investment property. This product offers access to home financing that advances your immediate and long-term financial goals.

What if I want to cancel my Personal Plan choices?
If you choose to cancel your Personal Plans benefit choices, you must submit the request in writing by completing the Personal Plans Cancellation Form.
