No Flu for You – Or Your Family Members
Primary Care Physicians
You and your dependent(s) who are enrolled in one of Banner’s medical plans may obtain flu shots and other required vaccinations through your Primary Care Physician(s) at any time during the year.
Mollen Clinics
You and your dependent(s) who are enrolled in one of Banner’s medical plans can receive the vaccine through the Mollen Clinics nationwide, at no cost to you. Mollen will submit the claim for your vaccination on your behalf. Visit the Mollen Immunization Clinic website for a location near you. Mollen is also able to provide the nasal flu vaccine, please contact them to obtain a listing of the locations that will have this type of vaccine. Adult patients may download the Mollen Vaccine Screening Form and fill it out in advance of their shot. The clinic staff will determine which vaccine is best for each patient based on age, etc. If you have family members who need to get a flu vaccine but are not covered by a Banner medical plan, they can get their flu shots at the Mollen Clinics for $35.00.
Other Locations
If you or your dependents choose to receive a vaccine at another location (CVS, Walgreens, Fry’s Pharmacy, etc.) you may submit a Medical Services Reimbursement form to your Banner medical plan for reimbursement of up to $25.
