Qualified Event

• Your legal marital status - marriage, divorce, annulment, legal separation or the death of a spouse or for domestic partner end of relationship.
• Your number of dependents - birth, adoption or placement for adoption or the death of a dependent.
• You or your spouse’s or domestic partner’s employment status (this means gain or loss of eligibility for regular full-time to less than regular part-time or vice versa or begins or returns from an unpaid leave of absence.
• Your unmarried dependent child’s eligibility status - satisfying or failing to satisfy a Plan’s requirements regarding age or student status.
• You or your spouse or domestic partner become Medicare eligible.
• Your transfer of residence or work site that creates a change in your health plan coverage (from an HMO to an indemnity or vice versa).
