Financial Assistance Programs

 

Banner Financial Assistance Programs:


The Basic Financial Assistance Program provides qualified uninsured patients with discounted hospital charges for medically necessary services. Patients may qualify for assistance if annual family income is less than $125,000.  Participants in the Basic Financial Program will be given a discount from hospital charges.  In most cases, the discount results in patients paying less for medically necessary hospital services than most commercial health insurance companies.  Patients must submit, at a minimum, an affidavit verifying annual household income and such other information as may be reasonably requested by Banner with respect to annual income, assets and liabilities. 


The Enhanced Financial Assistance Program: For uninsured patients who are unable to afford the Basic Financial Program and need additional assistance, Banner offers the Enhanced Financial Assistance Program.   Patients may qualify for the Enhanced Financial Assistance Program if they have annual household incomes less than 500 percent of the federal poverty guidelines. In this program, patients receive substantial discounts for medically necessary hospital services. The amount of the discount varies based upon the household income level; eligible patients below 150 percent of the federal poverty guidelines may receive free care for medically necessary hospital services.

In order to participate in the Enhanced Financial Assistance Program, patients must complete a Financial Assistance Application and submit a copy of the prior year’s federal income tax return, current bank statements and pay stubs (if employed).  In Arizona, patients must also apply for AHCCCS and be denied before becoming eligible for the Enhanced Financial Assistance Program. 
  
Payment Arrangements –Banner Health also offers no-interest payment arrangements. 

These discounts do not apply to services that offer packaged rates such as bariatric surgery or obstetrics.
