Tell A Friend –
Physician Referral Program
Everyone deserves a great physician. If you are happy with your Banner physician and our practice, we invite you to share your experience with a friend. Use this page to refer your Banner physician to a friend, and enjoy your complimentary $10 Starbucks Card on behalf of Banner Health.

How to participate:

Step 1: Your role: As a current patient of one of our participating physician offices please print the selected practice page and use the page to identify which physician you are referring and fill in your name as “referring patient”. Pass the card along to a friend.

Step 2: Your friend’s role: If your friend makes an appointment with a Banner physician practice, he or she will fill in their name and turn the page in at their appointment.

Step 3: Collect your reward: Once your friend’s first appointment is completed, we will mail you a complimentary $10 Starbucks Card, on behalf of Banner Health, to thank you for your referral.

We encourage you to refer your physician as many times as you like.
