Community Based Physicians


Community-based physicians are invited to join the Banner Gateway Medical Center Medical Staff via a unique, new membership category.  The Community Based Physician category is open to physicians who do not treat patients at the medical center but who request medical center services for their patients or require access to their patients’ health information.  This category allows physicians in the community to apply for membership on the medical staff without privileges.

This group of physicians is represented within the formal medical staff via an organized medical staff committee with a chairperson who has representation on the Medical Executive Committee.  

Community Based physicians may apply for membership by requesting an application from the Banner Credentialing Verification Organization.  There is a one time application processing fee.  This application can also be used for future membership to other Banner Health organizations. 

Initial Application Request

Community Based Physicians can:

  • Serve on the community based physician committee
  • Order outpatient diagnostic services for patients
  • Make courtesy visits to their patients
  • Be appointed to other committees unless otherwise provided in the bylaws
  • Vote on matters presented at committee to which they have been appointed and unless otherwise limited in the bylaws
  • Attend general staff meetings
  • Attend medical center continuing education programs
  • Utilize Medical Library Services (national literature searches)
  • Utilize the Medical Staff lounge to network with other members of the medical staff.
