Connecting communities to care

Community Healthcare Workers (CHWs) are trusted frontline professionals who bridge the gap between providers and the communities they serve. They bring cultural insight, lived experience, and strong community connections to support whole-person care.

To address the ongoing workforce crisis and rising patient demand, we are partnering with Pipeline Connects, Arizona State University, behavioral health providers and community organizations to expand the role of Community Healthcare Workers. This collaborative approach builds a sustainable workforce pipeline while extending care beyond traditional settings.

What we do

HeartHealth Promotion: Conduct education on nutrition, disease prevention (e.g., heart disease), and wellness

NavigationNavigation & Advocacy: Help individuals navigate healthcare systems, find resources for transportation, housing and food

Heart in handDirect Support: Health screenings (like blood pressure checks), and case management

WorldCultural Mediation: Provide translation and ensure services are culturally and linguistically appropriate

Core roles

1. Cultural Liaison and Trusted Messenger

  • Bridge between underserved communities and health/social service systems
  • Ensuring culturally appropriate and responsive care

2. Health Education and Coaching

  • Chronic disease management and prevention
  • Hygiene, nutrition, and healthy behaviors
  • Workshops and direct coaching

3. Patient Navigation and Care Coordination

  • Navigating complex healthcare systems
  • Scheduling appointments and reducing barriers
  • Facilitating access to social services

4. Outreach and Community Engagement

  • Reaching residents where they live, work, and play
  • Identifying health issues and promoting healthy lifestyles

5. Direct Services and Screenings

  • Basic health screenings (blood pressure checks)

6. Advocacy

  • Individual and community-level advocacy
  • Improving access to insurance and social assistance
