Being diagnosed with diabetes, or experiencing complications from this condition, can be life-altering. Our diabetes home care educators provide the in-home support you need to adjust your new normal. We help you gain the skills and knowledge necessary to successfully manage your diabetes independently, including medication therapy and lifestyle changes.

What Is Diabetes?

Diabetes is a chronic disease that occurs when you have too much glucose, or sugar, in your blood. Diabetes affects your body’s ability to properly convert food into energy, or glucose, and affects your insulin levels, which help get glucose to your cells.

Diabetes can be effectively managed with medications and lifestyle changes. If left untreated, diabetes can cause health problems such as stroke, heart disease, blindness, wounds, infections, kidney damage and nerve damage.

What Services Are Included?

Banner Health’s home diabetes care program provides a variety of services to our diabetic patients and their caregivers. We can help you manage your diabetes, monitor your health and prevent or treat complications.

Every patient receives customized diabetes education, tools and training to care for their diabetes at home. We’re here to provide as much support as possible to enhance your well-being and help you gain control of your health.

Medications

In addition to administering medications, our nurses teach you and your caregivers signs and symptoms to look out for to treat hyperglycemia (blood glucose too high) and hypoglycemia (blood glucose too low).

Nutrition and Fitness

Our care team can help you apply lifestyle changes as prescribed by your doctor. In general, doctors recommend diabetics follow a healthy diet low in carbohydrates and sweets and get at least 30 minutes of aerobic exercise every day to lower blood sugar.

Blood Glucose Monitoring and Education

Self-monitoring your blood glucose level is an important part of your diabetes home care. Banner Health’s nurses can help you check your blood sugar and teach you how to do it as well.

Foot Education

Patients with diabetes should get regular foot exams to avoid complications from the disease. Our diabetes educators can teach you how to prevent foot complications and spot potential concerns.
