In-Home Nutrition Consultations

If you have a chronic condition or are recovering from an illness, a carefully planned diet can make a big difference in your health. Banner Health’s qualified, friendly home care team includes dietitians who can provide in-home nutritional counseling, education and assistance to patients and their caregivers.

What Is Nutritional Consulting?

Our home nutritional counseling includes programs, screenings and support to help patients eat better, live healthier, manage chronic diseases or recover from serious illnesses. Our registered dietitians provide a number of nutritional support services in the comfort of your own home. Each patient’s nutritional counseling plan is tailored to fit their individual needs to stay healthy, reach a healthy weight, incorporate a physical activity routine and improve overall well-being.

We take an integrated, holistic approach to care, which means nutritional counseling may be added to your care plan as one tactic to help you manage your condition. What Does a Nutritional Consultation and Assessment Include?

Banner Health’s home nutrition support can help you achieve a healthier lifestyle. The first step is a nutritional consultation, where your dietitian will get to know you – including your goals, medical history, lifestyle, stress levels, sleep patterns, exercise routine, food preferences and cooking habits.

Next, we complete a comprehensive evaluation of your diet, weight and body mass index to determine your individual nutritional needs.

From there, we work with you to build a nutrition plan, including strategies to improve your diet and tips for meal planning and grocery shopping. Please be sure to ask us any questions you may have.

Ongoing nutritional counseling services may be done   via email, video visit or phone.
