CLAS Standards in Action

Culturally and Linguistically Appropriate Services (CLAS) standards guide our approach to equitable care. Serving a diverse membership, we ensure services respect and respond to varied cultural and linguistic needs.

Identifying Health Disparities

Through comprehensive sub population analyses, we stratify data by race, ethnicity, language and other key factors to identify care gaps and health disparities. These findings are summarized in our annual Health Disparity Report, which guides the development of targeted interventions.

Targeted Programs

Our Health Disparity Incentive Program promotes health equity addressing specific disparities within sub-populations including age, race/ethnicity, gender/gender identity, serious mental illness (SMI) and counties. The program targets specific measures such as:

  • Breast Cancer Screenings (BCS)
  • Cervical Cancer Screenings (CCS)
  • Oral Evaluation/Dental Exams (OED)
  • Colorectal Cancer Screenings (COL)
  • Glycemic Status (GSD): HbA1c/GMI Poor Control
  • Controlling High Blood Pressure (CBP)
  • Well Child Visits (WCV)

Proactive Member Outreach

We employ multiple engagement strategies to reach high-risk and low-adherence populations:

  • Text messaging campaigns
  • Educational mailers
  • Targeted phone outreach through Care Transformation Specialists and Population Health Assistants

These efforts provide education, scheduling assistance, In home test kits, and personalized support to overcome barriers to care.
