Basic operating skill and high degree of manual dexterity
Good hand-eye coordination with excellent vision and spatial awareness
Excellent communication skills to manage a wide variety of patients and families
Ability and willingness to accept constructive criticism
Aptitude to work as a team, take initiative, and lead effectively where needed
Provide compassionate and quality patient care

The application criteria for this fellowship include:

Graduate from Accredited NP/PA training Program
Pass appropriate certification examination
AZ Licensure/AZ Licensure-Eligible
Active BLS & ACLS Certifications
Eligible for DEA number
Personal Statement: one-page typed statement on why you are interested in postgraduate surgical training program
2 Professional Letters of Recommendations (including one from your graduate program director if you graduated within the last 2 years)
Curriculum Vitae/Resume
Official Transcripts from Graduate Program
Physician Assistants will require official NCCPA Exam scores
Nurse Practitioners must submit Adult Gerontological Acute Care NP Certification and Registered Nurse First Assist Certification
