Banner – University Family Care/ACC (B – UFC/ACC) cares about the health of our members. Completing wellness exams can prevent many health problems and benefit your child’s life. Our rewards program is directed toward improving the health outcomes for your child to prevent risk of more complicated issues occurring. We want your child to have the best quality health care possible.
Q: What is the Well-Baby Visit Campaign?
A: This incentive offers up to a $30 reward to parents or guardians of B – UFC/ACC members who attend their child’s appointments and complete the required visit/exams between 15-30 months of age.
Q: Who is eligible?
A: Any eligible parent or guardian of a B – UFC/ACC member, between 15- and 30-months of age, who attend an EPSDT / Well-Child visit (maximum of two) within this age range.
Q: Where can the reward be used?
A: Parents or guardians of members can shop in-store at any of the approved stores in-network, including Walmart, Albertsons, Safeway, Kroger, Fry’s, Walgreens and CVS. For a full list of stores nearby, members can always visit the andmorehealth.com website or “andmore” mobile app for more information.
Q: Is there a store finder website/app members can access to view participating stores?
A: Yes, andmorehealth.com gives parents or guardians of members access to all their benefits, balances, transactions and additional resources like a store finder for ease of card use at one of our approved stores in network.
Q: When should the rewards arrive?
A: To receive the reward, there will need to be a claim filed with the date of your exam/visit sent from your provider. Please allow six to eight weeks after your eligible visit for delivery of your B – UFC/ACC &more Benefits Prepaid Mastercard® in the mail with your rewards preloaded.
Q: Will lost or stolen reward cards be replaced?
A: Yes, if you have a lost or stolen card, please call 855-263-6673, TTY 711.
Q: Is there a charge for a replacement rewards card?
A: No, lost and replacement card orders will only be processed at the beneficiary’s request or at the member’s request. Parents or guardians of members may incur an added fee for expedited shipping of lost or replacement cards or new kits.
Q: Is there a dormancy fee for not using the card?
A: No.
Q: Does the rewards card expire?
A: Yes, the rewards card will need to be used within one year of the issue date.
Q: Are there any other fees?
A: No, there are no additional fees to the member.
Q: Can the card be used at an ATM?
A: No, there is no ATM access.
Q: How to activate the rewards card?
A: Please follow the instructions that came with the card or call 855-263-6673, TTY 711.

Q: Are there restrictions with my purchases?
A: This rewards card will not allow the purchase of products such as lottery tickets, tobacco, alcohol, firearms or gift cards.
Q: Is there a limit of rewards per household?
A: No, each household member that is eligible for the reward and has completed their visit/exam between 15-30 months of age will be eligible for the reward card.
Q: Can one household member be eligible for multiple rewards?
A: Yes, if the member is eligible for more than one reward.
Q: If one sibling is eligible and another sibling has already received a wellness visit previously, will they be considered for the reward?
A: No, each household member that is eligible for the reward and has completed their visit/exam between 15-30 months of age will be eligible for the reward card.
Q: Does the member need to provide confirmation that a wellness visit was completed?
A: No, B – UFC/ACC will receive a claim from the provider upon completion of the wellness visit.
