The AHCCCS Housing and Health Opportunities (H2O) Program is focused on Medicaid members with a Serious Mental Illness who are experiencing homelessness or housing instability and have an identified H2O qualifying chronic health condition or have experienced justice involvement within the last 90 days. The H2O Program will provide housing supports and integrated health care services to qualifying members.
The H2O Program provides the following services if eligible:
Banner – University Family Care/ALTCS (B – UFC/ALTCS) is committed to helping eligible H2O members receive covered H2O services through the contracted network of H2O providers through Solari Inc. B – UFC/ALTCS will assist H2O providers in coordinating care for eligible members and connect H2O providers to the members behavioral health team when assistance is requested. B – UFC/ALTCS will designate a point of contact for each eligible H2O member ensuring H2O providers receive timely assistance and members receive quality care.
