Need Help Renewing or Applying for ALTCS?

Our Community Assistors are approved by AHCCCS and can provide guidance at no cost.

B – UFC/ALTCS Community Assistors can assist with:

• ALTCS applications
• Renewal (redetermination) processes
• Reporting changes to address, income or household information
• General questions about ALTCS eligibility and more.

What is a B - UFC Community Assistor?

A Community Assistor is a trained B – UFC/ALTCS staff member approved by AHCCCS who can help you complete your application or renewal.

They can guide you through the process and help make sure your information is submitted correctly.

What You'll Need for Your Appointment with a B – UFC/ALTCS Assistor

Consent Form Requirement

Before we can assist you, AHCCCS requires your consent. You can provide it in one of the following ways:

• Verbally during your appointment with a B – UFC/ALTCS Assistor
• Download and complete the consent form below, then email it to [email protected]
  • Assistor Consent Form     English | Español
• Request a mailed copy by filling out the B – UFC/ALTCS Assistor Request Form

Whether you are applying for the first time or renewing your benefits, you may need one or more of the following pieces of information to complete your application:

• Proof of citizenship for everyone who is applying for benefits.
• Alien Registration Cards, if there are non-U.S. citizens applying for benefits in your household.
• Social Security numbers for everyone, or proof that a Social Security number has been applied for.

Having these documents ready will help speed up the process and prevent delays.

Who May Qualify for ALTCS

ALTCS provides services for individuals who may need long-term care support.

You may qualify if you:

• Are age 65 or older, or have a disability
• Need assistance with daily activities such as bathing, dressing or mobility 

and

• Meet medical and financial requirements
